Purpose:

  1. Need to increase capacity for Marion County’s outpatient substance use treatment and
    behavioral health services for uninsured, homeless and working poor
  2. Community need to increase accessibility and capacity for:
    a. Outreach & Navigation
    b. Assessments
    c. Counseling / Intensive Outpatient Therapy
    d. Peer Support Activities, AA, NA, 12-Step
    e. Medication Management & Medication Assisted Treatment (MAT)
    f. Case Management
    g. Medical, Dental and Vision
    h. Life Stabilization & Self-Sufficiency Services
  • Education, GED, Trade Skills
  • Job Readiness and Employment
  • Case Management for Occupational, Social, Educational, Emotional, Relationship and Essential Housing Stability
  • Obtaining benefits for which the client is eligible (e.g., VA, Medicaid, Social Security, etc.)

Goals & Outcomes:

  1. Reach people in the early stages of addiction before OD – dental clinics, ER’s, referrals
  2. Reduce Arrests, ER visits, OD’s and impact on jail/court system relating to SUD/MH
  3. Stabilize & Reduction in Harm
  4. Reduce Frequency & Impact of Relapse
  5. Build a centralized continuum of care on one campus
  6. Create a Sense of Hope, Self-Esteem and Sense of Community
  7. Projected Outputs (First Year)
  • 1,800 Substance Abuse/Behavioral Health Outreach Assessments
  • 10,000 uninsured, homeless and working poor clients served
