Choices, Chances, Changes

Our primary purpose is to provide a safe, non-judgmental forum in order to reduce resistance where cognitive behavioral interpretations, reviews, and appropriate behaviors can be implemented. In order to raise a coordinated community response towards relapse prevention or recidivism, and to nurture solutions encompassed within the constructs of Choices, Chances, Changes programs. Facilitators will continue to learn and remain ever- educable in the field of mental health and substance abuse and the process of helping clients to extinguish old patterns by implementing new and appropriate skills. Victim empathy and safety will be fostered as well as change encouraged.

COMMUNITY COMPANION CARE

After years of experience with our own family, we decided to put a team together with the express purpose of working with the elderly and disabled adults. We work closely with physicians, and healthcare professionals, as well as legal guardians and healthcare surrogates. Most seniors would like to remain in their home as long as possible, and our services can be used to help achieve that goal. We have the very best non-medical services available.

If you, or your loved one, is trying to make that transition, but you are not yet sure, allow us to act as a liaison for families from afar. We can continue our service for you even after you go into an Assisted Living Facility or Nursing Home, by making motivational visits, and (officially) unscheduled checkups including a progress report back to distant families. We only hire the most compassionate companion care providers, and each one is carefully screened for safe driving records, a drug test, and a background check. An agency representative will follow up after each appointment to be sure you are happy with all our services.

Diabetes Empowerment Education Program (DEEP) @ Department of Health

Unlike many health conditions, diabetes is managed primarily by the person it affects, with support from a healthcare professional. Lifestyle changes, such as shifting to a healthier diet and exercising regularly, can reduce the risk of diabetes.

But becoming smarter about diabetes and its causes, effects, and treatments is vital to avoiding the risk.

DOH-Marion offers diabetes self-care education classes to help residents control their diabetes. The Diabetes Empowerment Education Program (DEEP) helps participants understand how diabetes works, realize the benefits of meal planning and exercise, and identify and prevent diabetic complications. The goal is to help participants reduce their diabetes risk by making small lifestyle changes. Classes meet once a week for eight weeks at locations around Marion County.

If you are interested learning more about DEEP or signing up for classes, please contact DOH-Marion at 352-629-0137.
